My cousin has rectal cancer. He began having very bad pain about six weeks ago. He is currently using 75 mcg fentanyl patches, but he still is in a lot of pain. He has some 30 mg morphine tablets. He wants to know if he can use one 30 mg morphine tablet with his patches. He says that the fentanyl patches really only work well for two days. His fentanyl patches are to be used for three days, and then on the fourth day he puts on a new patch. The thing is, he has bad breakthrough pain even with the fentanyl. His doctor told him to go off the MS Contin and just use the patches. However, even when he used the fentanyl patches AND the 30 mg morphine tablet, he still had pain. I need an opinion, please, of what would be best for him based on the information I have provided.

Yes, the person can definitely use the 30mg IR. It's very normal to use morphine with fentanyl. But it's important for you to know that your cousin is not using the patches correctly. As I recall, they are supposed to given 6 hours to be effective, and they last for 3 days. So, you put one on 6 hours prior to the time you need it, which means you shouldn't wait for the 4th day, but the 3rd day. The instructions are very specific and the doctor should know them.
